Under the Fair Credit Reporting Act, an employer must obtain a written consent from the applicant and provide the applicant with a clear and conspicuous written disclosure that a background report may be requested. The disclosure must be provided in a stand-alone document to prevent it from being buried in an employment application. Below is a sample release that will be customized for your company.

REQUIRED STEPS

1. Complete and return the forms listed in Section 1.
2. Review Fair Credit Reporting Act requirements.
3. Obtain customized release forms from Lagniappe.
4. Obtain written consent from candidate.
5. Fax or email candidate release forms and other paperwork to Lagniappe.
